We provide the highest standard of client-centered home support services available, and offer our clients trust, respect and dignity. Our services include house keeping, meal preparation, personal care and complex care depending on your needs and requirements.

Pre Service Assessment

Pre Service Assessment / Consultation Living Well offers a wide range of home care services for older adults. The assessment process helps us to meet client expectations by customizing our services to the needs of each client. Before the start of each service, a Living Well Care Manager visits each client’s home to meet with the client and family to determine the needs of the client and the family expectations. At this time, a safety assessment of the home is conducted and feedback is given to the family. ​ This pre-assessment service is provided free of charge to all Living Well clients. Periodic updates are conducted at least twice a year to ensure that clients are receiving appropriate care.

Hourly Services

Designed for clients who can still look after themselves, but require assistance with daily chores and homemaking activities.

​These activities may include:

  • Laundry
  • Meal planning and preparation
  • Shopping (if authorized)
  • Housekeeping
  • Dishes and cleaning
  • Watering plants (if authorized)
  • Garbage disposal
  • Pet care (if authorized)

Designed for clients living at home who can do some tasks on their own, but require assistance with certain daily living activities. These services are often used by clients recovering from surgery, stroke and other chronic diseases. Our home support workers and Care Managers go through regular checklists to ensure that clients are managing adequately at home. ​

These daily activities may include:

  • Bathing and bathroom routines
  • Dressing and personal grooming
  • Nutrition management
  • Mobility assistance
  • Transfers / lifting
  • Exercise routines
  • Administration of medication
  • Equipment management

Designed for clients living on their own or living in a facility who needs some added support. ​ These activities may include: Community outings,  reading,  games,  crafts, personal interests, walks, family events, medical or other appointments

Specialized Services

Our qualified caregivers provide specialized services for the following conditions:

  • Alzheimer’s / Dementia – personal care provided by caregivers specially trained in looking after clients with Alzheimer’s disease or Dementia.
  • Complex Care – personal care for clients with multiple medical conditions who require constant monitoring and clients who have recently been discharged from the hospital.
  • Palliative Care – personal care provided by caregivers specially trained to look after end-of-life needs.
  • Respite Care – a temporary caregiver is provided for periods ranging from a few hours to a few days or longer. Respite care is designed to give a regular caregiver (often a spouse or family member) a break from constantly caring for a family member.
  • Transitional Care – a caregiver works with a client who is leaving a medical facility and returning home. This helps to provide a smooth transition back to the home environment and prepares the client to become independent again. Alternatively, we can provide care while you are waiting for placement in a facility or provide companionship when you are in the facility.
